Top Hospital (Gold), Value Hospital (Basic+), Intermediate Hospital (Basic+), Public Hospital (Basic+) , Singles and Couples Cover (Basic+) and Premium Extras and all Care and Support Extras products are no longer sold as a new product. If you already have one of these policies, there's no change, and we'll still honour all of the conditions, inclusions and benefits you enjoy on the policy. However, if you're on a different level of cover and wanted to change, or already hold it and wanted to recommend it to a friend, these policy types are no longer available.

If you've been referred by a health care professional to participate in a health management program for a specific medical condition, you'll need to complete a Health Management Program Benefit Approval Form and submit this with your claim.
